Optum Case Interview: Complete Prep Guide (2026)

Author: Taylor Warfield, Former Bain Manager and interviewer

Optum case interviews are used to evaluate candidates for consulting, advisory, and strategy roles within one of the largest healthcare companies in the world. If you have an upcoming Optum interview, this guide covers the full interview process, how to solve Optum cases step by step, the most common behavioral questions, and practice cases you can use to prepare.

But first, a quick heads up:

McKinsey, BCG, Bain, and other top firms accept less than 1% of applicants every year. If you want to triple your chances of landing interviews and 8x your chances of passing them, watch my free 40-minute training.

What Is Optum?

Optum is a health services and innovation company that is part of UnitedHealth Group, one of the largest healthcare organizations in the world. With more than 300,000 employees operating across 150 countries, Optum plays a major role in how healthcare is delivered, managed, and paid for in the United States and globally.

Optum is organized into three core business segments:

  • OptumHealth: Provides direct care delivery and health management services. OptumHealth operates one of the largest care delivery networks in the U.S., serving roughly 103 million people as of 2025.

  • OptumInsight: Offers data analytics, consulting, technology, and managed services solutions to hospitals, physicians, health plans, governments, and life sciences companies. This is where most consulting and advisory roles sit.

  • OptumRx: Manages pharmacy benefits and delivers integrated pharmacy care services for over 60 million members.

Optum Advisory Services is the consulting arm within OptumInsight. Consultants in this group work directly with healthcare clients to solve complex business problems involving strategy, operations, analytics, and technology. The firm also runs a Consulting Development Program for recent graduates who want to build a consulting career in healthcare.

According to UnitedHealth Group's financial reports, Optum generated over $227 billion in revenue in 2024, making it significantly larger than most standalone consulting firms. This scale gives Optum consultants access to one of the largest healthcare databases in the world, which is a major differentiator from traditional consulting firms.

What Does the Optum Interview Process Look Like?

The Optum interview process for consulting and advisory roles typically includes three to four stages. According to Glassdoor data from 2026, the average hiring timeline is about 25 days from application to offer, and candidates rate the overall interview difficulty at 2.79 out of 5.

Here is what the typical process looks like:

Stage

Format

Duration

What Is Tested

Application screening

Online submission

1-2 weeks

Resume, qualifications, experience fit

Recruiter phone screen

Phone or video call

20-30 minutes

Motivation, background, role fit

First round interviews

Video or in-person

45-60 minutes each

Behavioral questions, case interview or scenario analysis

Final round interviews

In-person or panel

Half day to full day

Case interviews, behavioral depth, leadership assessment

About 65% of candidates on Glassdoor describe their Optum interview experience as positive. The process can vary depending on the specific role and business unit, but consulting and advisory positions almost always include at least one case interview or scenario-based problem.

What Are Optum Case Interviews?

Optum case interviews are structured problem-solving exercises that simulate real consulting work. They are most commonly used for roles in Optum Advisory Services, the Consulting Development Program, strategy positions, and product management roles. If you are unfamiliar with case interviews, check out our complete case interview guide for a full walkthrough.

During an Optum case interview, the interviewer will present you with a business problem. You will then work together for 30 to 45 minutes to analyze the problem and develop a recommendation. The problems are almost always rooted in healthcare because that is Optum's entire focus.

Optum case interviews differ from cases at firms like McKinsey, BCG, and Bain in a few important ways:

Dimension

Optum Cases

MBB Cases

Industry focus

Almost exclusively healthcare, health insurance, and life sciences

Any industry (retail, tech, energy, etc.)

Case style

Scenario-based, often data-heavy with applied analytics

Hypothesis-driven, structured problem solving

Healthcare knowledge

Helpful and sometimes expected

Not required

Difficulty level

Moderate (2.79/5 on Glassdoor)

Very high (3.5-3.9/5 on Glassdoor)

Data component

May include Excel exercises or data interpretation

Typically verbal with some math

The biggest takeaway is that Optum cases are healthcare-specific. If you are also preparing for healthcare consulting case interviews at other firms, much of the industry knowledge will transfer directly.

What Are the 6 Steps to Solve Any Optum Case Interview?

The process for solving an Optum case interview follows the same general structure as solving any consulting case. Having coached hundreds of candidates through healthcare cases, I recommend the following six steps.

Step 1: Understand the Case Background

The interviewer will read you a description of the business problem. Take careful notes on who the client is, what industry segment they operate in, and what objective you need to address. Pay close attention to healthcare-specific details like the type of payer, provider, or patient population involved.

The most important thing in this step is to confirm the objective. Addressing the wrong problem is the fastest way to fail a case interview, regardless of the firm.

Step 2: Ask Clarifying Questions

After the interviewer finishes, ask one to three questions to fill in any gaps. Good clarifying questions for Optum cases might include asking what type of health plan the client manages, whether the focus is on commercial or government (Medicare/Medicaid) populations, or what metric defines success for the engagement.

Avoid asking questions that are too specific at this stage. You will have time to dig deeper later.

Step 3: Structure a Framework

Ask the interviewer for a minute or two to organize your thoughts, then build a framework with three to four major areas you want to investigate. For a complete guide on how to build custom frameworks, see our article on case interview frameworks.

For Optum cases, your framework should almost always include at least one healthcare-specific element. For example, if you are asked to help a hospital system reduce costs, your framework might include: patient volume and revenue trends, cost structure (labor, supplies, overhead), operational efficiency (bed utilization, length of stay), and competitive and regulatory factors.

Do not use a memorized framework. Interviewers can tell when candidates are recycling generic structures, and it signals a lack of critical thinking.

Step 4: Start the Case

Once you present your framework, the interviewer will either direct you to a specific area to explore (interviewer-led) or ask you to pick where to start (candidate-led). Most Optum cases lean toward being candidate-led, which means you need to drive the conversation.

Pick the area of your framework that you believe is most important to the case objective. State why you want to start there and what question you are trying to answer.

Step 5: Solve Quantitative and Qualitative Questions

The bulk of the case will involve answering a mix of quantitative and qualitative questions. Quantitative questions might involve calculating profitability for a care delivery model, sizing a patient population, or interpreting claims data. Qualitative questions might ask you to brainstorm strategies for improving patient outcomes or evaluate a proposed partnership.

For every quantitative problem, lay out your approach before doing any math. For qualitative questions, use a simple structure (such as internal versus external factors or short-term versus long-term) to organize your answer. Always connect your findings back to the case objective.

Step 6: Deliver a Recommendation

At the end of the case, summarize your recommendation clearly. State your answer first, then provide two to three supporting reasons based on evidence you uncovered during the case. Finish by mentioning what you would investigate next if you had more time.

In my experience at Bain, the candidates who stand out are those who deliver a confident, structured recommendation even when the data is incomplete. Optum interviewers are looking for the same quality.

What Healthcare Knowledge Do You Need for Optum Case Interviews?

Unlike generalist consulting firms, Optum expects candidates to have at least a basic understanding of how the healthcare industry works. You do not need to be a clinical expert, but you should know the key players, payment models, and terminology. This is similar to the knowledge expected at other life sciences consulting firms.

Here are the most important healthcare concepts to review before your Optum interview:

Concept

What You Should Know

Payers vs. Providers

Payers are insurance companies (e.g., UnitedHealthcare, Aetna). Providers are hospitals, clinics, and physicians who deliver care.

Fee-for-service vs. value-based care

Fee-for-service pays providers per procedure. Value-based care ties payment to patient outcomes and quality metrics.

Medicare and Medicaid

Medicare covers adults 65+ and some disabled individuals. Medicaid covers low-income populations. Both are government-funded programs.

Pharmacy benefit management (PBM)

PBMs negotiate drug prices and manage prescription benefits for health plans. OptumRx is one of the largest PBMs in the U.S.

Population health management

Using data and analytics to improve health outcomes across a defined group of patients, often by identifying high-risk individuals.

Claims data

Records of healthcare services billed to insurance. Optum has access to one of the largest claims databases in the world.

HEDIS measures

Healthcare Effectiveness Data and Information Set. A standardized set of quality metrics used to compare health plan performance.

Medical loss ratio (MLR)

The percentage of premium revenue a health plan spends on medical claims. The ACA requires a minimum MLR of 80-85%.

You do not need to memorize every detail in this table. But having a working understanding of these concepts will help you ask better clarifying questions, build more relevant frameworks, and demonstrate genuine interest in healthcare consulting.

What Types of Cases Does Optum Ask?

Based on candidate reports from Glassdoor and other interview databases, Optum case interviews tend to focus on real-world healthcare business problems. The most common case types include:

  • Profitability and cost reduction: Help a hospital system, health plan, or care delivery network identify why costs are rising or margins are declining, and recommend solutions.

  • Market entry or expansion: Evaluate whether a healthcare company should enter a new market, launch a new service line, or expand into a new geographic region.

  • Operational efficiency: Analyze bottlenecks in clinical workflows, patient throughput, claims processing, or care coordination and recommend improvements.

  • Data and analytics strategy: Determine how a client should use data, predictive models, or analytics platforms to improve population health outcomes or reduce waste.

  • Pricing and reimbursement: Help a payer or provider organization design pricing strategies, evaluate reimbursement models, or negotiate contracts.

Practice Case 1: Hospital Profitability Decline

Your client is a mid-sized hospital system with 12 facilities in the Midwest. Over the past three years, operating margins have dropped from 8% to 2%. The CEO wants to understand what is driving the decline and what the hospital should do to restore profitability.

A strong framework for this case would include: revenue analysis (patient volume by service line, payer mix shifts, reimbursement rate changes), cost structure (labor costs, supply chain, overhead), operational metrics (bed utilization, average length of stay, readmission rates), and external factors (regulatory changes, competitive dynamics, population health trends).

Practice Case 2: Health Plan Market Entry

A large national health plan is considering entering the Medicare Advantage market in three new states. They want to know whether this expansion is worth pursuing and what their go-to-market strategy should be.

Consider the market attractiveness (eligible population size, growth rate of Medicare Advantage enrollment, competitor presence), the client's capabilities (provider network readiness, claims processing infrastructure, regulatory approvals), the financial model (expected premiums, medical costs, administrative costs, time to breakeven), and risk factors (regulatory risk, adverse selection, network adequacy requirements).

Practice Case 3: Population Health Analytics

An integrated health system wants to reduce emergency department visits among its highest-risk patients. They have asked Optum Advisory to build a predictive analytics model that identifies patients at risk of avoidable ED visits. How would you approach this engagement?

Think about what data sources you would use (claims history, clinical records, social determinants of health), what features would predict ED utilization, how you would validate the model, and what interventions the health system could deploy once high-risk patients are identified. For more healthcare-specific practice cases, see our guide on healthcare consulting case interviews.

What Behavioral Questions Does Optum Ask?

In addition to case interviews, Optum interviews always include behavioral or fit questions. These questions assess your motivation, teamwork, leadership, and cultural alignment with Optum's values of integrity, compassion, and innovation. For a broader list, check out our article on consulting behavioral interview questions.

Based on candidate reports, here are the 10 most common behavioral questions at Optum:

  • Why do you want to work at Optum?

  • Why are you interested in healthcare consulting?

  • Tell me about yourself and walk me through your resume.

  • Describe a time you worked on a team to solve a complex problem.

  • Tell me about a time you used data to make a decision.

  • Describe a situation where you had to manage conflicting priorities.

  • Give an example of when you had to influence someone without direct authority.

  • Tell me about a time you failed and what you learned from it.

  • How do you stay current with developments in the healthcare industry?

  • What questions do you have for me about Optum?

If you want to feel fully prepared for the behavioral portion of your interview, my fit interview course covers 98% of consulting fit interview questions and can be completed in just a few hours.

For the "Why Optum?" question, have at least three specific reasons. You could mention Optum's unique position at the intersection of healthcare and technology, the scale of data and analytics resources available to consultants, or the opportunity to make a direct impact on patient outcomes through advisory work. Avoid generic answers that could apply to any company.

How Should You Prepare for Optum Case Interviews?

Having coached thousands of candidates for consulting interviews, I recommend a structured four-week preparation plan for Optum. This timeline works whether you are a complete beginner or have some case interview experience already.

Week 1: Learn Case Interview Fundamentals

Start by learning the core structure of a case interview and the strategies for each step. If you want to learn quickly, my case interview course covers everything you need in as little as 7 days. Focus on learning how to build custom frameworks, solve quantitative problems, and deliver structured recommendations.

Week 2: Build Healthcare Industry Knowledge

Spend time learning the healthcare concepts in the table above. Read about Optum's three business segments and review recent news about UnitedHealth Group. Understanding the difference between fee-for-service and value-based care, how health plans work, and what population health management means will give you a significant advantage over candidates who only prepare generic cases.

Week 3: Practice Cases Solo and With a Partner

Do three to five cases on your own using the practice prompts in this article, then find a partner and do five to ten cases together. Focus on healthcare-themed cases whenever possible. After each case, spend at least 15 minutes reviewing feedback and identifying areas for improvement.

Week 4: Mock Interviews and Final Review

If possible, do one to two mock interviews with someone who has consulting experience. This is the most effective way to simulate the real interview environment. If you do not have access to a former consultant, interview coaching can help you improve up to 5x faster than practicing on your own.

In your final week, also prepare your behavioral answers and research Optum's latest news, acquisitions, and strategic priorities. Being able to reference specific company developments during your interview shows genuine interest and preparation.

How Much Do Optum Consultants Get Paid?
